Ottawa Health Research Institute provides a free resource for patient decision aids. In addition to providing generic forms that help assess your decision making needs, plan the next steps, and track your progress in decision making, they provide an extensive A-Z list of disease-specific decision aids, which provide data to weigh the pro’s and con’s of specific options. These decision aids have been reviewed to ensure their quality using the International Patient Decision Aid Standards.
The Foundation for Informed Medical Decision Making and, its partner Health Dialog, work with health plans and employers to provide shared decision programs that help patients clarify their options and the risks, benefits and preferences associated with each.
Dartmouth-Hitchcock Center for Shared Decision Making provides one-on-one counseling and a Decision Aid Library to support shared decision making with healthcare providers. In addition to providing a link to the Ottawa Health Research resource, the Decision Aid Library offers a wide array of educational videos and DVD’s created by Health Dialogue, which can be sent to you on loan. They are able to offer this service to the public free of charge, in part through financial support provided by the Foundation for Informed Medical Decision Making and Health Dialog.
